Conduct community outreach to promote healthy behaviors and connect individuals with medical services and social resources. Provide advocacy, informal counseling, and health education while maintaining detailed client records and monitoring treatment progress.
Assist individuals and communities to adopt healthy behaviors.
Conduct outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and community health.
May provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs.
May collect data to help identify community health needs.
By doing this, the Community Health Outreach Worker helps their organization fulfill its mission to service the community.
High school diploma/GED equivalent required.
Preferred with experience in community setting and linking patients to various community resources and experience with connecting patient to community resources.
Individuals with lived experience are encouraged to apply.
Bachelor’s degree in a Social Sciences field such as Sociology, Psychology or Social Work is preferable.
Bilingual and multilingual capabilities are preferred.
Field based experience required.
Valid Driver’s license.
Requires a high school diploma or GED and a valid driver's license, with a bachelor's degree in Social Sciences preferred. Field-based experience and familiarity with recovery processes and Narcan distribution are highly valued.
