Complete an initial, comprehensive and ongoing assessments to determine home health and/or hospice needs.
Develop patient focused care plans that establish goals, based on nursing diagnosis and incorporates palliative nursing actions.
Provide respectful, compassionate care in patients’ homes.
Act as Case Manager and assume responsibility to coordinate patient care for an assigned caseload.
Collaborate with interdisciplinary team members.
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
