About the role

The Clinical Case Manager provides clinical services to help patients meet bio-psycho-social needs through therapeutic treatment, crisis intervention, and family therapy. They are also responsible for managing treatment plans, coordinating legal paperwork, and leading psychotherapeutic groups.

Responsibilities

  • We are currently hiring a Per Diem Clinical Case Manager for Fremont Hospital.
  • The Clinical Case Manager is responsible for performing clinical services functions in assisting patients in meeting their bio-psycho-social needs.
  • These include enabling the patient, through therapeutic treatment skills, to achieve their optimal level of emotional health.
  • Services provided are basic consultation, teaching, specialized clinical services for patients/families receiving care, crisis intervention, short-term family therapy, and providing information and referrals as needed.
  • In addition, the Clinical Case Manager is responsible for managing the treatment activities offered to the patient including: the level system; completing the psycho-social forms for each new patient and entering specific information on the treatment plans; interfacing with the physician led treatment team; and provision of psychotherapeutic groups and activities to patients and their families.
  • The Clinical Case Manager provides oversight and coordination of legal paperwork and LPS regulations.

Qualifications

Education

Bachelor’s degree for an accredited school of Social Services, Psychology, Nursing, Social Work, or Mental Health related field required. Master’s Degree is preferred.

Experience

  • Minimum 2 years direct clinical experience in a psychiatric or mental health setting and experience in patient assessment, family motivation, treatment planning and communication with external review organizations or comparable entities is preferred.
  • Licensure/Certification: Masters level may be actively pursuing and registered for professional licensure by The California Board of Behavioral Sciences preferred (MFTi or ACSW). Requirements

A Bachelor's degree in Social Services, Psychology, Nursing, or a related field is required, with a Master's degree preferred. Candidates should have at least 2 years of direct clinical experience in a psychiatric or mental health setting.
