Join a collaborative, patient-focused clinical team delivering comprehensive acute and chronic care management for adult and geriatric patients in a rehabilitation hospital. You'll partner closely with supervising physicians and interdisciplinary care teams to drive real improvements in patient outcomes and quality of life, all on a predictable Monday–Friday, 7am–5pm schedule in Oak Creek, WI.

  • What You'll Do
  • Provide primary care for adult and geriatric patients with acute and chronic conditions
  • Perform comprehensive assessments, establish diagnoses, and develop individualized treatment plans
  • Prescribe medications and order/interpret diagnostic tests within your scope of practice
  • Coordinate care and clinical decision-making with facility teams and supervising physicians
  • Maintain accurate, timely, and thorough clinical documentation
  • Collaborate with specialty providers (e.g., wound care, psychiatry) for integrated care delivery
  • Communicate clearly with patients, families, and care teams on care plans and changes in condition
  • Build trusted, lasting relationships with patients, families, and facility staff

  • Active Nurse Practitioner or Physician Assistant license in good standing
  • Active DEA registration
  • Current BLS certification
  • Strong diagnostic and clinical reasoning skills
  • Ability to work independently in a structured care environment

Requirements

Requires a valid Wisconsin license as an Advanced Practice Nurse Practitioner or Physician Assistant and BLS certification. A postgraduate degree in Nursing or a professional degree in Physician Assistant studies is required, with a preference for one year of Internal Medicine experience.
