True Care is currently seeking multiple Per Diem Fee-for-Service Registered Nurses (RNs) to join our growing clinical team. This field-based position is responsible for conducting in-home patient assessments throughout all New York boroughs and Westchester County while ensuring patients receive high-quality, compassionate care.
We are specifically seeking nurses with evening and weekend availability to support our expanding after-hours and weekend assessment coverage. This role is ideal for experienced RNs looking for flexibility, autonomy, and the opportunity to make a meaningful impact in home care.
Compensation $100 per Start of Care (SOC) Assessment $90 for all other assessments
Essential Responsibilities Conduct comprehensive in-home patient assessments, including Start of Care (SOC) and other clinical assessments as assigned.
Accurately evaluate patients' medical conditions, functional status, and home care needs.
Complete all required clinical documentation in a timely and accurate manner.
Develop and update patient plans of care in accordance with agency policies and regulatory requirements.
Collaborate with physicians, clinical leadership, Home Health Aides (HHAs), Personal Care Aides (PCAs), and the interdisciplinary care team to coordinate patient services.
Educate patients and caregivers on disease management, medications, safety, and home care services.
Report significant changes in patient condition to the appropriate clinical personnel.
Ensure compliance with all agency, state, federal, and Department of Health regulations.
Deliver compassionate, patient-centered care while maintaining professionalism during every patient interaction.
Active New York State Registered Nurse (RN) license in good standing. Minimum one (1) year of nursing experience preferred; home care experience is a plus. Strong clinical assessment, critical thinking, and documentation skills. Ability to work independently and effectively manage a field-based schedule. Willingness to travel throughout New York City's five boroughs and Westchester County. Availability to work evening and weekend shifts is required. Bilingual in Spanish, Creole, or other languages is highly preferred. Excellent interpersonal, communication, and organizational skills.
