Andrea's Angels is currently seeking multiple Per Diem Fee-for-Service Registered Nurses (RNs) to join our growing clinical team. This field-based position is responsible for providing skilled nursing visits and conducting in-home patient assessments throughout Greeley and Northglenn, Colorado, while ensuring patients receive high-quality, compassionate care. We are seeking experienced, compassionate nurses looking for flexibility, autonomy, and the opportunity to make a meaningful impact in home healthcare. This role offers per-visit compensation and flexible scheduling.

Compensation is fee-for-service and varies based on visit type and volume. This is a per diem position and is not eligible for employee benefits.
Essential Responsibilities Provide skilled nursing visits in accordance with the patient's plan of care.
Conduct comprehensive in-home patient assessments, including Start of Care (SOC) and other clinical assessments as assigned.
Accurately evaluate patients' medical conditions, functional status, and home care needs.
Complete all required clinical documentation in a timely and accurate manner.
Develop and update patient plans of care in accordance with agency policies and regulatory requirements.
Collaborate with physicians, clinical leadership, Home Health Aides (HHAs), Personal Care Aides (PCAs), and the interdisciplinary care team to coordinate patient services.
Educate patients and caregivers on disease management, medications, safety, and home care services.
Report significant changes in patient condition to the appropriate clinical personnel.
Ensure compliance with all agency, state, federal, and Department of Health regulations.
Deliver compassionate, patient-centered care while maintaining professionalism during every patient interaction.
Active Colorado Registered Nurse (RN) license in good standing is required. Minimum one (1) year of nursing experience preferred; home care experience is a plus. Strong clinical assessment, critical thinking, and documentation skills. Ability to work independently and effectively manage a field-based schedule. Willingness to travel throughout Colorado. Excellent interpersonal, communication, and organizational skills.
