What you'll do

As a Home Health Physical Therapist, you will

Perform comprehensive initial evaluations to assess patient needs and develop individualized treatment plans.

Design and implement evidence-based therapy programs to improve mobility, strength, balance, function, and overall quality of life.

Monitor patient progress, adjust treatment plans, and ensure optimal outcomes.

Educate patients and family members on exercises, safety techniques, and home programs.

Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.

Maintain accurate, timely clinical documentation in accordance with company policies and regulatory requirements.

Follow infection control and patient safety standards during every visit.

Deliver compassionate, patient-centered care while promoting our mission and values.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current, unrestricted Physical Therapist license in the state of practice.
  • CPR Certification.
  • Valid driver's license and reliable transportation with current auto insurance.
  • One (1) year of Home Health experience preferred.
  • Homecare Homebase experience preferred.
  • Physical requirements
  • Ability to stand, walk, sit, bend, kneel, squat, crawl, and climb throughout the workday.
  • Ability to safely lift, carry, push, and pull up to 100 pounds with or without reasonable accommodation.

Requirements

Must be a graduate of an accredited Physical Therapy program with a current, unrestricted state license and CPR certification. A valid driver's license and reliable transportation are required, with home health experience preferred.
