The Case Manager provides comprehensive support for residents in a recovery program, including intake, assessment, and goal planning for housing and employment. They also facilitate psychoeducational groups and coordinate medical care with the onsite clinic.
The Las Vegas Rescue Mission is looking for a Case Manager to join our recovery team.
LVRM is a private, faith-based organization that serves our community by offering a safe place for men and women to break the cycle of homelessness and addiction.
The Case Manager supports the ministries of the Las Vegas Rescue Mission by providing case management for residents of the recovery program.
The Residential Program is tailored to meet the needs of the individual and is based on principles from the Bible, relapse prevention, and the 12 steps.
The Case Manager assists with the coordination of client’s intake, assessment, and engagement into the program.
Create case management goal planning and update accordingly (housing, education, employment, etc.).
Facilitate psychoeducational groups as assigned.
The Case Manager assists with coordination of medical care with onsite clinic and medication pass.
Minimum High School Diploma Previous Case Management Experience Preferred Previous experience working with vulnerable populations preferred Skills, Abilities, and Special Talents HMIS Data Entry/ EHR experience Experience using Windows, Microsoft Word and Excel. Crisis Intervention/ de-escalation Group Facilitation Motivational Interviewing/ Person Centered Practices Demonstrates behaviors aligned with LVRM employment policies and standards. Willing to be accountable and receive corrections. Excellent verbal, written interpersonal and organizational skills. Networking skills This is a full-time position, 40 hours per week, offering a competitive benefit package and paid vacation. Salary is commensurate with skills and experience.
A high school diploma is required, with previous case management and experience with vulnerable populations preferred. Candidates should be proficient in HMIS/EHR and possess strong crisis intervention and interpersonal skills.
