About the role

Perform PM&R consults and follow-up care for patients at skilled nursing facilities. Coordinate treatment plans with facility staff, therapists, and supervising physicians while maintaining accurate clinical documentation.

We are seeking a full-time Nurse Practitioner (APRN) or Physician Assistant (PA) to join our growing Physical Medicine & Rehabilitation (PM&R) practice. The provider will work collaboratively with an assigned physician providing consultative and follow-up care primarily at skilled nursing facilities. This position offers a collaborative practice environment, scheduling flexibility, and the opportunity to work within a well-established PM&R team focused on high-quality rehabilitative care.

Work Location

Onsite, Tampa/Largo area.

Responsibilities

Perform PM&R consults and follow-up care at skilled nursing facilities independently and in collaboration with supervising physician Complete accurate and timely clinical documentation Review patient history, medications, and treatment plans Coordinate care with facility staff, therapists, and supervising physician Ensure compliance with clinical and documentation standards

Qualifications

Active Florida APRN or PA license Current DEA license (or eligibility) Experience in post-acute care, PM&R, orthopedics, neurology, pain management, or geriatrics preferred Strong clinical reasoning and documentation skills Ability to work effectively within a collaborative physician-led practice Reliable transportation for facility rounding Responsible for providing support at both the Tampa and Largo facilities, with travel between locations as needed. About Us Rehabilitation & Electrodiagnostics is a growing PM&R practice dedicated to providing rehabilitation and diagnostic services across hospitals, acute inpatient rehab facilities, skilled nursing facilities, and outpatient settings. Our team focuses on patient-centered care, collaboration, and clinical excellence.

Requirements

Must hold an active Florida APRN or PA license and a current DEA license. Experience in post-acute care, PM&R, or related specialties is preferred, along with reliable transportation for facility rounding.
