Responsibilities

  • Under the direction of the home care manager, the main function of the Occupational Therapist (OT) is to plan and provide occupational therapy program for treatment of patients in a home care setting, including but not limited to, practicing functional prevocational and vocational skills, activities of daily living and preparing progress reports.
  • Please review each essential job function: Makes initial and follow-up visits to patients assigned.
  • Uses the physical therapy process of assessment, diagnosis, implementation and evaluation to provide physical and psycho-social care to patients and their families.
  • Performs skilled care as indicated in the plan of care.
  • Develops individualized plan of care for patient under the order of a physician.
  • Maintains ongoing communication with care team and documents regularly against and appropriately updates care plans.
  • Collaborates with other health professionals, patient, and/or family in developing discharge plans.
  • Serves as an advocate for patient needs.
  • Instructs patient, family and/or caregivers in techniques throughout the episode of care, in preparation of discharge.
  • Manages work and patient schedule to make optimum use of time.
  • Adjusts schedule to accommodate unexpected occurrences and volunteers to assist others as needed.
  • Coordinates scheduling of treatments with other disciplines and follows department procedures when making appointments.
  • Leaves clear instructions for coverage on days off.
  • Complete documentation in a timely manner in accordance with agency policy.
  • Meets established productivity requirements.
  • Completes 30 day re-evaluations.
  • Follows departmental policy for planned and unplanned absences Presents in-services and participates in agency related education initiatives as assigned.
  • Accurately completes mileage and schedule in EMR as per agency policy.
  • Other duties within scope of job functions as assigned.

Requirements

Occupational Therapist license in Maryland Current CPR Certification Prior experience in home health preferred Valid driver’s license and reliable transportation
