Conduct initial and ongoing physical assessments to develop and implement personalized care plans for home health patients. Coordinate care as a Case Manager and maintain clinical documentation and communication with primary physicians.
Associate or bachelor’s degree in nursing from as accredited school of nursing. A valid/active RN license. Prior home health care or acute care experience preferred. Management experience preferred but not required. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Requires prolonged or considerable walking or standing and considerable reaching, stooping, bending, kneeling or crouching. Must be able to lift, position or transfer patients and lift supplies and equipment. Must have visual acuity and hearing to perform required nursing skills. Why Summit?
