About the role

The Speech Therapist conducts home visits to assess and treat IDD and geriatric patients according to a physician's plan of care. Responsibilities include developing treatment goals, documenting patient progress, and collaborating with healthcare teams to ensure continuous coordination.

Speech Therapist - IDD/Geriatric Home Health (Per Visit / Fee for Service, approx.

10 visits/wk) Preferred Certified is looking for an experienced and reliable home care Speech Language Pathologist to conduct home visits in Yonkers and the surrounding area for our IDD and geriatric patients.

The speech therapist contracted or employed through the organization provides speech therapy services to patients according to a written physician's plan of care.

This may include but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.

The ideal candidate would be comfortable with treating a pediatric population.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Assesses patient’s speech and language abilities and deficits.

Prepares an initial assessment of the patient’s condition and perform periodic reassessments.

Develops a plan of care in accordance with the authorized practitioner’s plan of treatment for therapy services and according to agency policies.

The plan of care must include the type of services rendered, an estimated duration of treatment, the frequency of visits with clearly defined, measurable short and long term goals.

Provides rehabilitative services for speech and language disorders to meet established treatment goals.

Selects and use appropriate diagnostic and therapeutic techniques and materials.

Documents type of treatment and patient’s response on designated clinical progress notes.

Complete cases conferences, prepare discharge summaries of service, and complete documentations on appropriate forms and as indicated.

Participates in the review, revision and implementation of patient care policies and procedures, relating to speech-language pathology.

Instructs other health care personnel, the patient and other care takers in methods to assist patient in home therapy programs to improve, correct, and/or accept the patient’s disability.

Participates in the development and periodic revision of the plan of care forms established for the patient.

Collaborates with other health care team members on as ongoing basis to ensure continuous coordination.

Participates in inter-disciplinary and case conferences, staff meetings and in service programs as required.

Participates in the agency’s Quality Assurance Performance Improvement/PIP as assigned.

Demonstrates sound judgment by taking appropriate actions regarding suspected violation of corporate compliance regulations.

Reports all suspected violations to supervisor, Compliance Officer or Compliance Hotline.

Performs other duties as assigned.

Position Requirements

A master’s degree and certification of clinical competence in speech pathology granted by American Speech language and Hearing Association A license and current registration to practice as a Speech-Language Pathologist in New York State. One full year of supervised institutional and one year home care experience is preferred. Meets state health status requirements.
