Become a part of Aliviane! Job Summary The Qualified Mental Health Professional will provide Targeted Case Management services to qualifying Center clients. Care Coordinator will be trained in and deliver family-oriented, individual and/or group psychosocial rehabilitative services, skills training, medication training, and/or case management services according to best practice standards set forth in the Texas Recovery & Resilience Guidelines. The Targeted Case Management Care Coordinator will provide services in a variety of community settings.
Candidates must hold a bachelor's degree in psychology, social work, or a related behavioral health field. Proficiency in both English and Spanish is required.
