Hospice Home Health Shift: 8-hr shifts, Days, 8 am – 5 pm Schedule: 5 X 8-hr shifts, M-F Position Requirements: · Position Requirements: · Registered Nurse: 1 year - Required · Recent/Current Hospice Nursing experience: 1 year with the last 3 years – Required · Home Care Home Base (HCHB) charting experience – Required · Must have valid Driver’s License and Provide proof of personal Auto Insurance · Must have reliable vehicle for patient visits · Must be Covid Vaccinated or provide medical or religious exemption · Education, License and Certification Requirements: · ASN or an equivalent – Required · BSN or an equivalent – Highly preferred · RN License for State applying or Compact if accepted - Required · BLS Certification from AHA or American Red Cross - Required ·
· Provide and coordinate care for assigned hospice patients in comfort of their homes. · Develop and implement nursing care plans including Start of Care and admissions as assigned. · Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy. · Empower patients and families to reach healthcare goals by educating them on disease management. · Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. · Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure. · Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented. · Notify physicians of significant changes in status. · Document and complete charting for all patient visits within 24 hours. · Provide bereavement support to patient’s families as needed. · Territories to cover: Milford area
Requires a Registered Nurse license with at least one year of experience in both general nursing and recent hospice care. Must be proficient in Home Care Home Base (HCHB) charting and possess a valid driver's license with a reliable vehicle.
