Interim HealthCare is seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract assignment serving patients throughout Dorchester, Charleston, and Berkeley Counties. Complete comprehensive patient assessments and ongoing evaluations.
Develop, implement, and update individualized plans of care in collaboration with patients, caregivers, and the interdisciplinary team.
Provide skilled nursing care as ordered by the physician.
Educate patients and families on disease management, medications, and health maintenance.
Perform preventative and rehabilitative nursing interventions.
Coordinate care with physicians and other healthcare professionals.
Monitor patient progress and communicate changes in condition promptly.
Supervise home health aides and other paraprofessional staff as appropriate.
Participate in Quality Assessment and Performance Improvement (QAPI) activities.
Complete accurate and timely clinical documentation.
Maintain HIPAA compliance and protect patient confidentiality.
