Under the direction of the Manager, LPN Supervisors, the LPN provides clinical and administrative support as a member of the behavioral health team in providing the highest quality of care to patients.
Duties/Responsibilities
Clinical Responsibilities
Explains procedures and treatments to patients to gain cooperation and provide understanding.
Works with the interdisciplinary health care team to facilitate care and keep patients engaged by positive communication and teamwork.
Administers treatments, nursing interventions, injections, and immunizations.
Reinforces patient education on self-injection, medications, or other treatments or issues related to care.
Participates in quality assurance and improvement activities related to improvement initiatives.
As needed, performs duties of Medical Office Assistant (MOA).
Collects samples from patients such as urine and performs tests such as EKGs as required.
Completes standardized screenings with patients such as the PHQ-9 and GAD-7.
Assists with managing MyChart messages and InBasket workflows, supporting both urgent and non-urgent patient outreach and messaging including medication questions, appointment related inquiries, and symptom or side-effect reports.
Routes messages appropriately to the Psychiatrist, Psychiatric NP, or Behavioral Health team based on established triage protocols.
Responds to patient messages within defined service standards using approved templates and clinical guidelines.
Administers psychiatric injectable medications per provider orders, scope of practice, and organizational protocols.
Performs medication administration documentation, monitoring, and follow-up as required.
Obtains and documents vital signs and relevant clinical information prior to psychiatry visits and/ or injections; updates patient information as necessary.
Manages medication refill requests including reviewing refill requests for completeness, verifying last visit/labs/monitoring requirements, and preparing refill requests for provider authorization.
Communicates with pharmacies regarding clarifications, prior authorizations, and refill issues.
Tracks controlled substance refill workflows in accordance with state and federal regulations and Trillium Clinic policies.
Completes and tracks psychiatric related paperwork including prior authorization forms, disability/FMLA forms as delegated, program or housing verification forms, lab orders and follow up tracking.
Coordinates labs, injections, follow-up visits, referrals, and external behavioral health services as needed.
Ensures accurate documentation in the EHR, including medication administration, patient communications, and care coordination activities.
Maintains compliance with all applicable regulations, including HIPAA, state nursing regulations, and organizational policies.
Participates in team meetings, trainings, and continuing education related to behavioral health nursing practice.
Practices within the NYS LPN scope of practice and maintains annual competencies.
Documentation
Documents patient care clearly, accurately, and timely according to policy.
Collects information concerning the health status of patients and document appropriately.

Required Skills/Abilities
The ability to demonstrate teamwork and effective interpersonal relations skills is imperative.
Excellent written/oral communication and computer skills are required.
Previous experience with an Electronic Health Record is desired.
Experience with EPIC EMR preferred.
Demonstrates excellence in communication, professionalism, stress management, and teamwork.
Expected to share knowledge ongoing across roles including orienting new nursing team members
Education and Experience
NYS LPN licensure with current CPR certification required.
Minimum 2 years with experience in psychiatry, behavioral health, or integrated primary care, required.
Experience working in community-based healthcare setting preferred.
Physical Requirements
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
