The Role

As a Licensed Practical Nurse (LPN), you will collaborate closely with our Nurse Practitioners. Your primary responsibility will be to provide holistic, high-touch, and coordinated care for a panel of senior patients, ensuring an exceptional patient experience.

Day-to-Day Responsibilities

  • Deliver outstanding clinical care and support functions related to patient care activities
  • Actively engage and manage patient panels in collaboration with the Nurse Practitioner, extending the delivery of high-touch, quality care
  • Ensure efficient clinic flow by rooming patients promptly and assisting the provider in maintaining a timely schedule
  • Conduct visit activities such as taking vitals, performing screenings, recording histories, and preparing patients for examination
  • Support post-visit activities, including facilitating referral completion, patient care coordination, and conducting follow-up outreach to patients as needed
  • Promote clinical and safety standards to drive high-quality care for our patients
  • Maintain a safe, secure, and healthy work environment by establishing and following standards and procedures
  • Under the direction of the Provider, offers LPN appropriate “visits”, such as administering vaccinations and performing procedures, including but not limited to: ABI, spirometry, EKGs, etc.
  • Oversees the laboratory and related activities including reviewing lab orders, drawing blood, collecting samples, and preparing specimens for laboratory shipment or analysis
  • Prepare, clean, and maintain instruments and equipment in the exam room and laboratory
  • Build a strong culture and foster community within our organization to create a premier patient and teammate experience
  • Support all clinical activities assigned by provider that are aligned with the scope of practice in the state of licensure

Experience and Qualifications

  • High School Diploma or GED equivalent
  • Graduate of an accredited Licensed Practical Nursing program
  • Valid LPN license
  • 3+ years of experience as a Licensed Practical Nurse
  • Primary Care experience is a plus
  • 1+ year of Phlebotomy experience (preferred)
  • Current BLS certification endorsed by the American Heart Association (Preferred)
  • 6+ months of laboratory experience (Preferred).

Requirements

Requires a valid LPN license, graduation from an accredited LPN program, and at least 3 years of experience as an LPN. Preferred qualifications include phlebotomy experience, BLS certification, and laboratory experience.
