About the role

Provide peer support services in the office or community for assigned clients and monitor care coordination activities. Prepare written documentation for services conducted and provide transportation to consumers as needed.

Description New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.

New Horizons is seeking a caring and compassionate Peer Specialist who is eager to learn how they might provide support to clients who need assistance as they learn and grow. This person believes that their efforts can make a difference in the lives of the people we serve. As a Peer Specialist your responsibilities will include

Provides peer support services either in office or in community for an assigned group of clients monitoring the activities of Care Coordination for high utilizing clients of New Horizons services Assists the Care Coordination Team with activities for assigned clients Prepares written documentation for each service conducted Provides transportation to consumers as needed

Requirements

New Horizons will request the following qualifications: Certified as a Peer Specialist or in process of Certification preferred Valid and current Florida Driver's License required with no more than 6 points in the past 6 years Must pass Level 2 Background through DCF and AHCA Clearinghouse.
