Description Are you a flexible, compassionate Registered Nurse who enjoys variety and thrives in a collaborative environment? Sangre de Cristo Community Care is seeking a Float RN to provide clinical support across our program. In this dynamic role, you'll help ensure continuity of care by assisting with admissions, patient visits, and providing coverage for RN Case Managers during PTO, education, and other staffing needs.

Completing hospice admissions and start-of-care visits Providing routine and PRN patient visits Covering RN Case Manager caseloads during PTO, leaves of absence, and education Assisting with symptom management and changes in patient condition Collaborating with physicians and the interdisciplinary team to develop and update individualized care plans Educating patients and families on disease progression, medications, and end-of-life care Supporting timely response to changing patient needs Accurately documenting all patient care in the electronic medical record Helping maintain excellent continuity of care across the team

Current Colorado Registered Nurse (RN) license Minimum one year of nursing experience required Hospice, Home Health, Oncology, Palliative Care, or ICU experience preferred Strong clinical assessment and critical thinking skills Excellent communication and organizational abilities Ability to adapt quickly to changing priorities and patient assignments Compassionate, patient-centered approach to care Valid driver's license, reliable transportation, and acceptable driving record
