Description Are you an experienced Hospice RN looking for exceptional work-life balance while making a meaningful difference? Sangre de Cristo Community Care is seeking a compassionate, independent Registered Nurse to join our team as a Resource RN.
This unique 7 days on / 7 days off position offers the opportunity to provide after-hours hospice care, admissions, and support to patients and families during their most critical moments, all while enjoying an entire week off between rotations.

As our Resource RN, you will serve as the primary after-hours clinical resource for patients and families by: Responding to urgent patient needs and symptom management calls Completing after-hours nursing visits Providing compassionate end-of-life support Performing hospice admissions when appropriate Pronouncing deaths and providing family support Coordinating care with physicians, the interdisciplinary team, funeral homes, and community partners Triaging phone calls and determining the appropriate level of intervention Accurately documenting all patient care in the electronic medical record Ensuring seamless communication with the daytime clinical team
