  • Summary: The Ambulatory Medical Home Pharmacist is responsible for providing comprehensive pharmaceutical care to patients in the primary care clinics. The Pharmacist evaluates, identifies and solves medication related problems and acts as a liaison between patients and patients' families and other health care team members regarding medication issues. Collaborates with physicians, nurses, and other healthcare professionals to ensure the delivery of safe, effective and economic drug treatment. Other duties as assigned.
  • Experience
  • DESCRIPTION: Minimum of two years of experience in the primary care or ambulatory internal medicine setting is required.
  • Education
  • DESCRIPTION: Bachelor's or Pharm.D. degree received from an ACPE accredited school of pharmacy is required.
  • Training
  • DESCRIPTION: PGY1 residency , PGY2 ambulatory care, primary care or internal medicine residency or equivalent experience is preferred.
  • SPECIAL
  • SKILLS
  • DESCRIPTION: Demonstrates effective use of computers and applications (e.g. MS-Word, Excel, Outlook) as appropriate to practice and communicate effectively.
  • Licensure
  • DESCRIPTION: State of Wisconsin pharmacist license (or eligible for) is required.

Requirements

Requires a Bachelor's or Pharm.D. degree from an ACPE accredited school and a Wisconsin pharmacist license. A minimum of two years of experience in primary care or ambulatory internal medicine is required.
