Description Prime Home Health and Companion Care is seeking compassionate Companion/Homemakers to support elderly, disabled, and chronically ill clients in their own homes. As a member of our team, you’ll companion and homemaking services and emotional support to create a safe, clean environment for each client.
Client Location: Westfield, MA (potential for additional shifts with clients in nearby areas)

Key Responsibilities Personal Care: Assist with daily activities, including bathing, dressing, grooming, and toileting.
Emotional Support: Provide companionship and emotional encouragement, helping clients maintain a positive outlook.
Mobility Assistance: Aid clients with mobility using Hoyer lifts and Sit-to-Stand devices (experience preferred but not required).
Follow Care Plans: Work closely with nurses, therapists, and doctors to provide care as outlined in individual care plans.
Maintain Environment: Keep the home environment clean, safe, and supportive of each client’s needs.
Accurately record patient care and report any changes in condition to the care team.
Certification: Current PCA and Homemaking certification.
Valid driver’s license, reliable transportation, and proof of auto insurance.
Prior experience working with elders is required.
