About the role

The LPN/RN provides client-centered services including care plan coordination, medication administration, and rapid HIV testing in a clinic setting. They are responsible for maintaining patient documentation, coordinating follow-ups, and collaborating with an interdisciplinary team to support patients living with HIV/AIDS.

Position Summary

The LPN/RN engages in client-centered services such as care plan coordination, treatment adherence, administering IM injections, follow up, and adherence to policies and procedures as required by Northland Cares, along with other local, state, and federal entities. The LPN/RN interacts with staff members of an interdisciplinary team in a clinic setting. Work Schedule & Exceptions

This is a PRN position. Specific location, days and hours are determined based on patient needs. Work is performed in either Cottonwood or Prescott Valley Northland Cares office hours are: Prescott Valley Monday through Thursday 8:30am to 4:30pm. Cottonwood Monday through Thursday 9:00am – 5:00pm Mileage Stipend for travel to Cottonwood Attendance is essential to this position; employees are expected to be available and ready to perform their duties during all scheduled working hours.

Minimum Qualifications

Must be a graduate from a nationally accredited nursing program and 1 year or more of applicable work experience in a medical setting including: giving injections administering medications Currently holds an unencumbered LPN or RN license with the AZ State Board of Nursing Must have an Arizona Driver’s License and up to date auto insurance Computer/technical literacy including general Microsoft Office and electronic health record (EHR) systems. Cultural and diversity Demonstrated ability to maintain confidentiality and HIPAA compliance. Current healthcare level CPR certification within 6 months of hire. Commitment to serving patients living with HIV/AIDS

Preferred Qualifications

Familiarity with HIV/AIDS services and Ryan White/HOPWA programs Experience in Health, Social Work, or related field Experience with Microsoft Excel Bilingual English/Spanish

Supervisory Reporting Structure

The LPN/RN reports to the Executive Director but also takes directions from the Medical Director. The LPN/RN collaborates with the Clinic Manager for patient care and quality management. SCOPE OF WORK

Assists with maintaining appointment schedules for patients requiring injections Coordinates continuous quality management data collection, monitoring, and reporting Documents actions in all required program databases and maintains confidential patient information, ensuring accuracy, completeness, and timely recording.

Facilitates clinical follow-up and referrals as directed by providers.

Responsible for cleaning and disinfecting exam rooms after use.

Prepares and provides patient medical documentation in compliance with all federal, state, and clinic policies and procedures.

Performs rapid HIV testing in compliance with manufacturer's instructions as needed Travel to work at other Northland Cares locations upon request as benefits Northland Cares.

Remains informed of patient status changes through communication with medical staff, laboratories, and case managers.

Ensure staff members are kept current on patient status for needed services and office workflow.

Interacts with patients, potential patients, and their families to advise on services offered by Northland Cares.

Contributes to team effort through task accomplishment, facilitation of communication with all clinic employees, and proactively advising supervisors of activities and needs.

Maintains security and confidentiality of client information, adhering to all principles of HIPAA.

Participates in meetings, as needed, to ensure excellent and efficient documentation, Quality Improvement, Program Management, and teambuilding.

Cross training with other staff as determined by supervisor.

Physical Requirements Sitting, standing, and walking throughout the day for short to moderate periods.

Driving distances of up to 2 hours for clinics, meetings, or training is required.

Travel to state and national conferences and training is required.

Regular use of office equipment such as computers, phones, printers, and copiers.

Occasional lifting of up to 50 pounds.

Requirements

Candidates must be a graduate of an accredited nursing program with an unencumbered AZ LPN or RN license and at least one year of medical experience. Required qualifications include a valid Arizona Driver's License, CPR certification, and proficiency in EHR systems and Microsoft Office.
