Must work well independently and in combination with a multidisciplinary team to provide efficient and effective treatment.
Assesses needs and works directly with the patient on the completion of treatment plan goals.
Provides counseling, education, and support to patients and their families in individual and group sessions.
Must have excellent written and verbal communication skills for documentation and therapeutic interaction purposes.
Interacts with Physicians and community agencies to provide continuity of care for the prevention of recurring problems and case management discharge planning.
Serves as a representative of the Behavioral Health Center by presenting community education programs.
This requires excellent organizational and presentation skills as well as extensive knowledge of mental health.
