Albany Home Health is seeking a compassionate and skilled Physical Therapist to provide in-home rehabilitation services that help patients regain mobility, improve strength, and maintain independence. Key Responsibilities
Develop and implement individualized treatment plans.
Perform patient assessments, including mobility, gait, strength, and range of motion evaluations.
Provide physical therapy services in accordance with physician-directed care plans.
Educate patients, families, and caregivers on therapy goals, exercises, and safety measures.
Monitor patient progress and maintain accurate clinical documentation.
Collaborate with physicians and interdisciplinary healthcare teams.
Participate in care conferences and discharge planning.
Supervise Physical Therapy Assistants and home health aides as required.
Identify additional patient care needs and coordinate appropriate referrals.
