About the role

Position Summary

The Licensed Vocational Nurse (LVN) / Medical Scribe is responsible for providing high-quality nursing support to medical providers and delivering exceptional patient care in an outpatient clinic setting. The primary function of this role is to perform licensed vocational nursing duties, including patient intake, clinical support, patient education, medication administration, and coordination of care. As the clinic workflow requires, this position will also serve as a Medical Scribe by documenting patient encounters in real time within the electronic health record (EHR), allowing providers to focus on patient care while ensuring accurate, timely, and complete medical documentation This position requires excellent clinical judgment, strong documentation skills, attention to detail, professionalism, and the ability to adapt quickly between nursing and scribing responsibilities. Essential Duties Licensed Vocational Nurse Responsibilities Prepare patients for provider visits by obtaining vital signs, medical history, medication reconciliation, and chief complaints. Assist providers during examinations, treatments, and minor procedures as needed. Administer medications, immunizations, injections, and treatments within the LVN scope of practice and organizational policies. Perform point-of-care testing, specimen collection, EKGs, wound care, and other clinical procedures as directed. Educate patients regarding medications, treatment plans, disease management, preventive care, and follow-up instructions. Respond appropriately to patient phone calls, electronic messages, refill requests, and provider instructions. Monitor patient flow to maximize clinic efficiency. Maintain accurate clinical documentation and nursing notes in the EHR. Maintain exam rooms, medical equipment, and clinical supplies. Ensure infection prevention, OSHA, HIPAA, and safety standards are consistently followed. Assist with quality improvement initiatives, population health activities, and care gap closure. Other duties as assigned.

Medical Scribe Responsibilities Accurately document provider-patient encounters in real time within the electronic health record.

Record histories, physical examinations, assessments, treatment plans, procedures, and provider instructions as dictated during patient visits.

Document laboratory results, imaging findings, referrals, and follow-up recommendations.

Prepare visit documentation prior to appointments by reviewing available patient records.

Assist providers with order entry, pending prescriptions, referrals, and documentation for provider review and signature when permitted by organizational policy.

Maintain strict confidentiality and accuracy of all patient records.

Ensure documentation supports coding, billing, regulatory compliance, and quality reporting.

Communicate effectively with providers to clarify documentation when needed.

Maintain productivity while preserving documentation quality and accuracy.

Education and Skills Current California Licensed Vocational Nurse (LVN) license in good standing.

Current BLS certification.

Graduate of an accredited vocational nursing program.

Minimum one year of outpatient clinical experience preferred.

Previous medical scribing experience preferred but not required.

Experience using electronic health records preferred.

Excellent typing, grammar, spelling, and computer skills.

Ability to multitask in a fast-paced clinical environment.

Excellent written and verbal communication skills.

Strong attention to detail and organizational skills.

Thorough knowledge of outpatient nursing practices.

Ability to accurately document complex medical encounters.

Strong knowledge of medical terminology, anatomy, pharmacology, and clinical workflows.

Strong understanding of HIPAA, infection prevention, and OSHA standards.

Ability to prioritize multiple tasks while maintaining accuracy.

Ability to maintain professionalism and composure under pressure.

Strong interpersonal skills with patients and interdisciplinary teams.

Commitment to patient-centered, compassionate care.

Physical Requirements Frequently stand, walk, bend, stoop, reach, and lift up to 25 pounds.

Ability to remain standing while documenting patient encounters for extended periods as needed.

Manual dexterity sufficient to operate computers, medical equipment, and office equipment.

Visual acuity sufficient to read electronic medical records and medication labels.

Requirements

Requires a current California LVN license, BLS certification, and graduation from an accredited vocational nursing program. Preferred candidates have outpatient clinical experience and proficiency with electronic health records.
