Senior Care Case Manager - LSW, LPC, MFT - Preferred
This role may be filled as either a part-time or full-time position.

Qualifications and Responsibilities of Senior Care Case Manager Must be at least 21 years of age Diploma or GED required Desire to improve quality of life for aging adults.
Must have experience working with Alzheimer patients.
High level of commitment, dependability, and interpersonal skills.
Prior experience as an STNA, CNA, or in a nursing or long-term care setting is preferred.
Prior experience in social work, counseling, human services, or a related field is preferred.
Valid driver’s license and reliable transportation with current automobile insurance meeting state minimum liability requirements.
Previous EHR system use, and professional documentation experience are helpful.
Candidates must have experience working with Alzheimer patients and possess a valid driver's license with reliable transportation. Prior experience as an STNA, CNA, or in social work and human services is preferred.
