The Licensed Social Worker MSW will initiate direct patient care services, family and patient counseling discharge planning, crisis intervention and consultation, coordinate referrals and promote educational programs. Required Educational Degree: Master’s Degree Major/Area of Concentration: Required Certifications, Credentials and Licenses: Current Social Work license from the state of Ohio Counselor and Social Work Board. Required Experience: 1 year experience in social work Preferred Experience: Previous home care experience preferred. . Experience in discharge planning for Care Management.
