The Case Manager performs comprehensive patient assessments and initiates discharge planning upon admission. They are responsible for evaluating, documenting, and updating the patient's Plan of Care based on clinical responses.

Case Manager The Case Manager is responsible for performing assessments appropriate to functional status, diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner.
The candidate will also identify and prioritize the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care.
The Case Manager initiates discharge planning for each patient upon admission as appropriate.
In addition, the Case Manager evaluates and documents the patient’s response to the Plan of Care and to interventions; revises and updates the Plan of Care as evidenced by documentation.
Qualifications

Requirements
Candidates must have a current California Registered Nurse license and completion of an accredited nursing program. Two years of acute care experience is required, with CCM or CPUR certifications preferred.
