Responsibilities

  • Provide physical therapy evaluation and treatment for residents in a skilled nursing setting
  • Develop and implement individualized treatment plans to address functional limitations and mobility
  • Perform therapeutic modalities, therapeutic exercises, gait training, and balance activities as indicated
  • Reassess progress regularly, document outcomes, and adjust plans accordingly
  • Collaborate with an interdisciplinary team to support discharge planning and ongoing rehabilitation goals
  • Maintain accurate clinical documentation and ensure compliance with regulatory and facility protocols

Requirements

  • Active physical therapy license (PA license or eligibility for PA) and ability to practice in the United States
  • Doctor of Physical Therapy (DPT) or equivalent professional degree
  • Relevant experience in a skilled nursing facility or similar setting preferred
  • Strong documentation and communication skills; ability to work within a multidisciplinary team
  • Ability to work in a long-term care environment and adapt to patient needs
