About the role

Conduct comprehensive patient assessments and develop individualized plans of care in collaboration with medical providers. Provide skilled nursing services, administer treatments, and coordinate care as a case manager within a home health setting.

Signature Healthcare at Home is seeking to add a Registered Nurse for Skagit County! This position would cover Skagit County (Mount Vernon, Burlington, Sedro-Woolley, Bow, Anacortes, Hamiliton, Concrete) for home health skilled nursing services. This position works Monday through Friday day shift, with self-directed scheduling dependent on patient needs and territory. Duties and Responsibilities

Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family/caregiver living situation to determine care needs. Provides a complete physical assessment and history of current and previous illness(es). Establishing the initial individualized plan of care in collaboration with the certifying medical provider based on comprehensive assessment and patient goals. Regularly re-evaluating patient and family/caregiver needs. Participating in revising the plan of care as necessary in collaboration with the certifying medical provider as patient status and needs change. Develops a care plan that establishes interventions and goals. Includes the patient and the family/caregiver in the planning process. Works autonomously to initiate appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the medical provider in the plan of care. Provides health care instructions to the patient as appropriate per assessment and plan. Facilitates the patient’s efforts toward self-sufficiency or obtaining appropriate caregiving. Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload. Documents patient services and status timely and accurately within professional practice standards and reimbursement requirements. Communicates with the medical provider regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives medical provider orders as required. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the medical provider and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary team. Attends required staff meetings, interdisciplinary team meetings and care coordination activities, completes mandatory education requirements to maintain competence. Coordinates patient care in tandem with LPN/LVN as assigned by Clinical Manager. Knowledge, Skills and Experience Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home health preferred. Currently licensed as a Registered Nurse in Washington State. Possesses and maintains CPR certification. Must be licensed and able to practice in Washington State. The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.

Preferred Qualifications

Clinicians with experience in Home Health or post-acute care. Familiarity with Homecare Homebase EMR. Schedule, Pay and Benefits Full-time (32+ hours a week) Monday through Friday. Hourly $50 - 55 (Dependent on Experience) + Mileage Reimbursement. Medical/Dental/Vision available the 1st of the month after hire. Paid company holidays, 2+ weeks PTO and 401k match after 90 days. Requirements

Must be a graduate of an accredited nursing school and a licensed Registered Nurse in Washington State with CPR certification. Preferred experience includes 1-2 years of acute care and 1-2 years of home health experience, along with a valid driver's license and vehicle.
