In this role, you'll help clients overcome barriers to recovery by coordinating services, connecting them with community resources, and advocating for their long-term success.

About Us Indiana Treatment Centers, in partnership with Ascension Recovery Services, provides evidence-based treatment for individuals experiencing substance use disorders and co-occurring mental health conditions. Our mission is to empower individuals on their journey to lasting recovery through compassionate, person-centered care in a safe and supportive environment.

What You'll Do As a Case Manager, you'll play a key role in helping clients successfully navigate treatment and transition into recovery by: Provide comprehensive case management services for clients with moderate to high clinical and psychosocial needs.

Coordinate discharge planning and transitions of care to ensure continuity of services.

Facilitate interdisciplinary case reviews and communicate client progress, barriers, and treatment recommendations.

Assist clients with accessing housing, employment, transportation, insurance, benefits, community resources, and recovery supports.

Build collaborative relationships with referral sources, community agencies, probation officers, courts, employers, and other community partners.

Advocate for clients while promoting accountability, self-sufficiency, and recovery.

Maintain timely, accurate, and compliant documentation within the electronic health record (EHR).

Participate in quality improvement initiatives, compliance activities, and multidisciplinary team meetings.

Support a positive, recovery-focused treatment environment.

Perform other duties as assigned.

What We're Looking For Required Qualifications High school diploma or equivalent required; associate's or bachelor's degree in a behavioral health or human services field preferred.

Minimum of one (1) year of experience providing case management, care coordination, or direct services in behavioral health, substance use disorder treatment, mental health, healthcare, or a related human services setting.

Knowledge of community resources and social service systems.

Experience documenting in an Electronic Health Record (EHR).

Current CPR/First Aid certification or ability to obtain within 30 days of hire.

Preferred Qualifications

Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field. Two (2)+ years of experience in residential or outpatient substance use disorder treatment. Training in Motivational Interviewing, Trauma-Informed Care, or Recovery-Oriented Systems of Care. CADAC, Indiana Certified Recovery Specialist (CRS), Certified Peer Recovery Coach (CPRC), Certified Case Manager (CCM), or other relevant behavioral health certification preferred. Knowledge of Indiana Division of Mental Health and Addiction (DMHA) standards and community behavioral health resources preferred.

Work Environment This position is primarily based within a behavioral health treatment facility and may include occasional travel within the community to coordinate services and connect clients with local resources. Team members work closely with individuals experiencing substance use disorders, mental health conditions, and complex psychosocial needs in a fast-paced, collaborative, and rewarding environment.
