New River Valley Community Services Medical Services is seeking a Case Manager! The Case Manager provides needs assessments, links resources, and monitors services for individuals with substance use and/or co-occurring disorders with the MAT (Medication Assisted Treatment) Team. Theis role coordinates services and resources for clients as needed to maintain in their communities; in addition to monitoring services as they are delivered to ensure consumers' needs are met. The Case Manager also ensures documentation process requirements are met accordingly along with assistance with transportation and community integration as needed.
$21.56-$24.79 - Depending on Experience
Additional compensation will be considered for any new employees that speak multiple languages.
