Responsibilities

  • Administer medications and treatments per physician orders under supervision
  • Monitor residents’ conditions, document vital signs and changes
  • Perform nursing assessments, develop and adjust care plans in collaboration with the interdisciplinary team
  • Provide direct patient care and assistance with activities of daily living
  • Assist with routine procedures, wound care, and infection prevention as directed
  • Communicate changes and outcomes to the supervising registered nurse and maintain accurate documentation

Requirements

  • Active LPN license with ability to practice in Vermont
  • Experience in long-term care or geriatric settings preferred
  • Strong assessment, documentation, and communication skills
  • Ability to work evenings and/or overnight shifts
  • Demonstrated reliability and ability to collaborate within a care team
