Responsibilities

  • Administer medications and treatments as prescribed and per facility policy.
  • Monitor resident conditions and promptly report changes to the supervising RN.
  • Perform routine nursing tasks, wound care, injections, vital signs, and patient documentation.
  • Assist residents with activities of daily living and mobility, ensuring safety and comfort.
  • Collaborate with the health care team to implement and update individualized care plans.
  • Document care accurately in the medical record and adhere to infection control standards.

Requirements

  • Active Vermont LPN license in good standing
  • Experience in a long-term care or geriatric setting preferred
  • Flexible to work evening and/or overnight shifts
  • Willingness to participate in a travel assignment
  • Strong communication and teamwork skills
  • Ability to follow care plans and maintain accurate documentation
