About the role

The Nurse Practitioner provides comprehensive primary care, including diagnosis and treatment of acute and chronic illnesses for medically complex elderly patients. They collaborate with an interdisciplinary team to develop care plans and conduct assessments in clinics, day centers, and homes.

Nurse Practitioner (NP) – PACE

HopeWest PACE is seeking Primary care or Geriatric experienced Nurse Practitioner to join a robust interdisciplinary team, to collaborate in developing and implementing comprehensive care plans while enhancing the lives of medically complex elderly individuals.

What is PACE?

PACE (Program of All-Inclusive Care for the Elderly) was created to answer the many problems around caring for frail and medically complex seniors. The PACE model is intended to keep individuals living safely and comfortably in their homes whenever possible.

HopeWest PACE participants receive wrap around services that include but not limited to in home care services, transportation to a HopeWest PACE center for primary medical care, social and recreational activities.

The Unique Responsibilities of the Nurse Practitioner at HopeWest PACE

  • Provides a wide range of Primary Care services including diagnosis and treatment of chronic and acute illness, preventative care, palliative, and urgent medical needs all in collaboration with the PACE Medical Director.
  • Conducts health assessments and evaluates patient needs in the clinic, the PACE Day Center and the patient’s home.
  • Actively collaborates with the entire PACE interdisciplinary team for the highest quality care of each participant.
  • Make appropriate and timely referrals to our contracted providers out in the community.

What You Bring

Two (2) years of Nurse Practitioner clinical experience preferred.

Experience working with older adults, individuals with multiple chronic conditions, or medically complex individuals.

Experience in primary care, hospice, palliative care, home health, long term care or PACE preferred.

Current and unrestricted APN and RXN license in Colorado required.

National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or other appropriate population focus preferred.

Controlled substance license with current DEA registration or eligibility to obtain.

Current CPR/BLS certification maintained throughout employment.

Requirements

Candidates should ideally have two years of clinical experience and a current APN and RXN license in Colorado. National certification as an FNP or AGPCNP and a current DEA registration are preferred.
