Senior Care Options by Parker provides customized Geriatric Care Management services as well as Behavioral Health services by licensed social workers to individuals in their homes and communities across Middlesex, Monmouth, and Ocean Counties. Program participants of both programs benefit from services offered in their own environment by our dedicated care partners who are committed to meeting their needs. Services will help support participants in maintaining independence in their own environment.
Requires a Master's degree, a valid LSW license in New Jersey, and a valid NJ driver's license. Two years of counseling experience and knowledge of EHR are preferred, and a flu vaccine is required during flu season.
