About the role

The Nurse Practitioner will provide palliative care through evaluation, assessment of health history, and diagnosing medical conditions. Responsibilities also include developing individualized plans of care, completing face-to-face assessments, prescribing treatments, and collaborating with the interdisciplinary team.

What You’ll Do

Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions.

Develop individualized plans of care.

Complete face to face assessments and prescribe medications or treatments.

Collaborate with the interdisciplinary team.

  • Active NP certification (state-licensed).
  • Valid driver’s license, reliable transportation, and current auto insurance.

Requirements
