The RN Hospice Case Manager performs comprehensive nursing evaluations, assesses for and manages patient discomfort, and initiates/manages the plan of care while instructing caregivers. This role also coordinates all services provided to patients and families to ensure quality of care.

We are seeking an experienced and compassionate Per Diem RN to join us in this mission.
RN Hospice Case Manager- Per Diem
Performs comprehensive nursing evaluations.
Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action.
Initiates and manages plan of care, provides appropriate nursing follow –up to all patients, and instructs caregivers in managing patient needs.
Coordinates all services provided to patients and families, as well as appropriateness of care, to ensure quality of care and services.
$50 - $60 per hour
