Provide case coordination and therapeutic support for children and families navigating mental health challenges. Develop care plans and collaborate with schools, healthcare providers, and community partners to improve service access.
Provide client care, support, and specific case coordination services to children, youth, and their caregivers with direct supervision for a caseload of 15-30+ clients. Develop and implement plans of care for a moderately complex/acute caseload. Complete basic client referrals via case presentation to the CCRC committee for personal caseload. Complete advanced client intake assessments, develop and implement treatment plan goals for a moderately complex/acute case load. Use advanced service/ treatment planning skills to maximize client engagement and actively manage caseload rotation. Develop and implement therapeutic group opportunities for children, youth and/or caregivers. Provide advanced support and case coordination to foster care providers, adoptive parents, kin caregivers and / or biological parents. Work directly with client and their care team to expand their access to and skillful utilization of their community support networks and coordinate referrals to longer-term services Establish and maintain collaborative working relationships with external agencies, including schools, hospitals, and other designated/social service agencies. Engage internal/ external service providers through case reviews, referral management, clinical consultation, care transitions. Develop and maintain working knowledge of WCMHS programs and services, with particular focus on those provided to children and families. Manage moderate problems and crisis situations with children, youth and families that may require a multi-person approach. Complete advanced and multi-systemic client referrals for personal caseload. QualificationsEducation and Experience Bachelor’s or master’s degree in psychology, social work, or counseling. 1 - 3 years of experience working within mental health with specific experience with children, youth and caregivers required. Experience coordinating between multiple services, programs, or agencies or equivalent skills to manage the communication and organization process of helping clients navigate a system of care. Demonstrate a basic understanding of trauma informed care practices. Knowledge, Skills, and Competencies Excellent organizational skills and attention to detail. Ability to interact well with parents, school staff, WCMHS staff, and other professionals. Valid driver’s license, excellent driving record, and access to a safe, reliable, insured vehicle. Ability to complete all physical requires of the job with or without reasonable accommodation. Drive own personal vehicle to and from a variety of agency locations and client homes.
Requires a Bachelor's or Master's degree in psychology, social work, or counseling with 1-3 years of mental health experience. Must possess a valid driver's license and a reliable insured vehicle.
