What You'll Do In This Role

Make a meaningful difference every day as a Certified Nurse Aide (CNA) in our PACE Day Center, providing compassionate, hands-on care and support to frail elderly participants. Working closely with the healthcare team, you will help carry out individualized care plans that promote comfort, dignity, independence, and overall well-being in a warm, participant-centered environment. Under the guidance of the Day Center Supervisor, this role offers the opportunity to build meaningful connections while making a lasting impact in the lives of older adults.

Key Responsibilities Provide Hands-On Care– Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.

Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.

Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.

Perform Vital Checks – Measure and record vital signs, weight, and intake/output.

Provide Responsive Support – Respond with kindness and compassion.

Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.

Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Skills, Knowledge & Expertise Must be certified as a Certified Nurse Aide in the State of Michigan.

High School Diploma or GED required.

Position requires the ability to lift and move objects weighting up to 35 pounds without assistance.

Must have reliable transportation.

Flexibility to provide coverage in the evening and on weekends.

Have one (1) year experience working with the frail elderly population.

Valid drivers license.

CPR certification is desired or obtained within 3 months of employment
