What You’ll Do

The LPN Home Care Coordinator is responsible for coordinating and supporting nursing services for participants in their homes and community settings. The LPN Home Care Coordinator completes participant visits, provides nursing support within the LPN scope of practice, communicates changes in participant condition, and helps ensure care plans and services are carried out effectively for our PACE participants.

Key Responsibilities Conduct in-home participant visits for routine, periodic, and acute assessments as directed.

Perform routine home and facility visits as assigned to deliver services and provide follow-up care based on participant needs.

Contribute to the development and ongoing revision of participant care plans.

Communicate clinical observations and changes in participant condition.

Provide direct participant care in the home setting, including but not limited to wound care, catheter changes and flushing, medication reconciliation, acute symptom management and triage, vital sign monitoring, and laboratory specimen collection, etc.

Initiate timely clinical interventions to promote and maintain participant safety.

Provide education to participants and caregivers regarding chronic and acute health conditions, treatment plans, and symptom management.

Assist in coordinating durable medical equipment (DME) needs within the home.

Participate in assigned nurse on-call responsibilities.

Attend IDT meetings, care planning meetings, team huddles, and care conferences.

Accurately and timely document care, plan variations, and observations in the Electronic Health Record (EHR).

Skills, Knowledge and Expertise Active Licensed Practical Nurse (LPN) licensure in the State of Michigan. Two (2) years nursing experience with at least one (1) year experience working in a skilled nursing facility setting or geriatric care. Must possess a valid, unrestricted state of Michigan driver's license. Must possess a current CPR certification.
