In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.
Key Responsibilities Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
Provide Responsive Support – Respond with kindness and compassion.
Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.
