What You'll Do In This Role

Are you a CNA looking for your next role? LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.

Key Responsibilities Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.

Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.

Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.

Perform Vital Checks – Measure and record vital signs, weight, and intake/output.

Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.

Provide Responsive Support – Respond with kindness and compassion.

Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.

Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Skills, Knowledge & Expertise Certified Nurse Aide (CNA) per state requirements, required. Previous experience providing care to older adults, preferred. Reliable transportation and valid driver’s license, required. High school diploma or GED, required. Excellent communication and observation skills.
