EPIC Health Partners is seeking a compassionate and motivated professionals to join our Mental Health Skill Building team. This position plays a vital role in promoting recovery, independence, and wellness—all under the supervision of a licensed mental health professional. Deliver person-centered care in homes and community settings. Assist in the creation and ongoing revision of service plans tailored to the needs of each individual. Monitor and ensure that service plans are being effectively carried out. Provide linkage to resources and case management support for individuals. Submit necessary documentation according to established timeframes. Maintain continuous communication with individuals and key stakeholders to ensure coordinated planning and service delivery. Participate in required training and team meetings.
Registered as a QMHP, QMHP-T with the VA Board of Counseling, or certified as a QPPMH Bachelor’s degree or higher in a Human Services-related field (e.g., Psychology, Social Work, Counseling). At least 1 year of behavioral health experience. Valid driver’s license and reliable transportation. Current CPR/First Aid certification and TB screening.
Field-based role requiring travel to client homes and community locations. Must be able to sit, stand, and engage with clients for extended periods. Flexible scheduling, including evenings or weekends, may be required.
