Works under the supervision of a Clinical Team Leader. Provides individualized patient care for patients in all developmental stages throughout the life span including Adult - 18-72 years and Geriatric - 72 + years, according to established policies, procedures, and guidelines. Performs patient evaluations and treatments according to procedural guidelines for home health patients referred to Occupational Therapy. Participates in professional responsibilities such as program development, student programs, and performance improvement programs. Provides patient consultation and communicates with other professionals. This position is responsible for the occupational therapy of patients in all stages of life in their home. Performs other appropriate duties as assigned. PRIMARY JOB DUTIES 1. Assesses, interprets, plans, implements, and evaluates patients with appropriate level of skill according to the patient’s age and diagnosis. 2. Assists with program development and discipline specific operations efficiently and productively. 3. Appropriately communicates information in accordance with agency policies and procedures and discipline-specific guidelines. 4. Contributes to program effectiveness. 5. Graduate of Occupational Therapy program from an accredited university. 2.
Licensure /
Certifications: Licensure in the state in which providing services (NC/SC); CPR certification, certification by AOTA. 3.
Must be a graduate of an accredited Occupational Therapy program with state licensure (NC/SC), CPR certification, and AOTA certification. A valid driver's license, operational vehicle, and the ability to lift over 50 lbs are required.
