Erie Family Health Centers is seeking a full time Community Health RN to provide essential clinical support to patient care activities utilizing nursing education, assessment, and procedural skills. Duties & Responsibilities

Collaborates with providers in the delivery of patient care. Performs triage assessment process of walk-in patients and telephone calls. Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching. Administers immunizations, applicable contraception refills, and medications as ordered by provider. Performs other duties as assigned. The work schedule for this role is aligned with Erie's hours of operations. QualificationsEducation Current Illinois RN licensure required. Current BLS for Healthcare Provider CPR certification required. One to two years’ experience in community health nursing or ambulatory care preferred. Skills and Knowledge Bilingual is a plus. Requirements

Requires a current Illinois RN licensure and BLS for Healthcare Provider CPR certification. One to two years of experience in community health nursing or ambulatory care is preferred.
