Licensed Social Workers & Mental Health Professional new hires are eligible for a $7500 retention bonus! As a Clinician in the Crisis Residence, you will provide short-term crisis assessment, crisis counseling, treatment planning with behavior management and clinical interventions that identify your client's emotional, behavioral, and psychiatric crisis. You will implement all assessments and treatments using a strength-based, trauma informed, systems oriented perspective and will include development of crisis prevention and safety planning for when the client is discharged back to the community . You will provide case management services such as coordination of services with existing treatment providers and linkage to treatment and any additionally community-based therapeutic resources and supports needed to assist with the transition back to the community successful. You will report to the Director for the Crisis Residence and will work onsite.

Responsibilities

Complete the intake process for admission to the crisis residence Risk Assessment Substance Abuse Screen/Assessment CSSR Social Needs Screening - Health Leads Comprehensive Psychosocial Assessment to include diagnosis and needs to develop treatment plan (IAP) Obtain release of information forms for all current past mental health providers, inpatient and outpatient, school, primary care provider Provide short-term crisis intervention and stabilization to youth while in the crisis residence. Conduct on going indicated risk assessments, mental status examinations. Formulate preliminary diagnosis (with appropriate supervisory sign-off) Analyze clinical information to identify crisis treatment plan to include behavioral management, stabilization, safety planning, and integration back into the community safely. Provide individual and group counseling. Facilitate and coordinate tele-mental health services. Facilitate family meetings to provide education, therapy and discharge planning. Consult with team and supervisors. Document information in accordance with applicable standards. Collaborate and coordinate with current treatment providers and facilitate linkage at time of discharge to include services such as Health Home, Outpatient and Inpatient Treatment Providers (Mental Health and Substance Abuse), Medication Management, and Primary Care. Monitor implantation and progress of treatment plan goals and adjust as needed to work towards stabilization and return to the community. Point person for all community providers of youth admitted to Crisis Residence Provide short-term follow up at discharge to determine any further needs for continued stabilization in the community. Provide education support to the Residential staff caring for the youth in the residence. Engage in advocacy for the clients and families served in the crisis residence. As assigned rotate schedule to provide coverage during holidays, vacations and sick calls. Hours will be flexible to meet program, client and family needs Requirements and Education: Requires a Master’s degree and three to four years of related experience. Requirements

Requires a Master's degree and three to four years of related experience. Preference is given to candidates with LCSW, LMSW, or LMHC licenses, though MSW/MHC-LP is considered.
