Provide psychosocial assessments, counseling, and crisis intervention for cancer patients and their families. Coordinate with interdisciplinary teams to manage discharge planning, patient assistance programs, and community referrals.

BSW as a minimum qualification but strongly preferred is an MSW, At Least One Year of Professional Experience in Hospital or Health Related Setting. Expertise With Data Management Tooks. Adherence To National Association of Social Workers Code of Ethics.
1.Provide quality social work assessment and intervention to meet the psychosocial needs of patients of SCTC departments and their families and significant others.
2.Enable the patient and family to understand and utilize to the fullest extent services and education provided by the cancer center and/or other agencies and resources and to be knowledgeable regarding cancer and the specific disease process.
3.Enhance the overall functioning and coping of the patient and his/her family within the community through a counseling relationship.
4.Work with the inpatient Case Manager/ Discharge Planner to assure appropriate and timely completion of the discharge planning process for all patients who have discharge planning needs.
5.Makes any necessary referrals for the patient and his/her family in collaboration with other interdisciplinary team members as needed, with appropriate follow-up.
6.Plan and implement any necessary support groups, educational activities, and celebrations for cancer patients and their families.
7.Provide social adjustment, supportive, and crisis intervention counseling for all cancer patients and their families, as needed.
8.Interact daily with assigned Physicians and Midlevel providers in order to identify potential barriers and suggest interventions to minimize treatment delays, assure patients are treated at the most appropriate level of service, and assure cost effective treatment.
9.Document all social work activities.
10.Other duties as assigned by the SCTC Manager or SCTC Director.
11.Age Specific Competency: Staff members will have knowledge of human growth and development through the life span of the patient and have the ability to meet the needs of and provide the care of these patients.
The age categories to be included are: Adolescence 13 - 17 yrs; Young Adult/Adult 18 – 64 yrs; Adult/Middle-Aged 65 - 79 yrs; Geriatric 80+ yrs 12.Works closely with financial navigator, oral chemotherapy nurse, and pharmacist to assist with patient assistance programs as needed.
13.Assist with locating and applying for grants from outside sources to fund patient needs to fund identified patient needs.
14.Act as a liaison between the Scotland Cancer Treatment Center and community agencies (ie Hospice of Scotland County, DSS, etc).
