This role is ideal for clinicians who value hands-on, relationship-driven work and want to see the real-life impact of their care every day. What You’ll Do Provide community-based mental health and substance use services to children and families Conduct comprehensive assessments to guide individualized care Develop and implement recovery-focused service plans with clear, measurable goals Deliver evidence-based, brief, and solution-focused therapy (individual and family) Support families through case management and care coordination as part of a multidisciplinary team Meet clients where they are—in homes, schools, and community settings Participate in team staffing and treatment planning meetings twice weekly Maintain timely, accurate documentation in accordance with agency standards Why This Role Matters You’re not just providing therapy—you’re helping families build stability, strengthen relationships, and create lasting change.
