About the Role

As a LVN/RN at Smoky Hill Health & Rehabilitation, you will play a critical role in delivering compassionate and high-quality nursing care to our patients.

Your primary focus will be to support the healthcare team by providing direct patient care, monitoring patient health, and assisting with medical procedures under the supervision of registered nurses and physicians.

You will ensure that patients' physical and emotional needs are met, contributing to their overall well-being and recovery.

This role requires meticulous attention to detail, strong communication skills, and the ability to work collaboratively within a multidisciplinary team.

Ultimately, your efforts will help maintain a safe, supportive, and healing environment for all residents and patients in our facility.

Minimum Qualifications

  • Valid and current Licensed Vocational Nurse (LVN) or RN license in the state of Kansas.
  • High school diploma or equivalent.
  • Completion of an accredited vocational nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated ability to provide compassionate patient care.

Preferred Qualifications

  • Experience working in a long-term care or rehabilitation facility.
  • Additional certifications such as IV therapy or wound care.
  • Familiarity with electronic health record (EHR) systems.
  • Strong interpersonal and communication skills.
  • Ability to work flexible shifts including weekends and holidays.

Responsibilities

  • Provide direct nursing care to patients, including administering medications, monitoring vital signs, and assisting with daily living activities.
  • Observe and report changes in patients' conditions to the nursing supervisor or physician promptly.
  • Assist with wound care, catheter care, and other medical procedures as directed by healthcare professionals.
  • Maintain accurate and detailed patient records, documenting care provided and patient responses.
  • Collaborate with interdisciplinary team members to develop and implement individualized care plans.
  • Educate patients and their families on health maintenance, disease prevention, and post-treatment care.
  • Ensure compliance with healthcare regulations, safety protocols, and infection control standards.
